Questions You May Be Asked After You Are Discharged

  1. Was your orientation to the rehab unit adequate?
  2. Did you receive adequate notice of your discharge?
  3. Were your concerns addressed quickly?
  4. Did the rehab unit provide a safe environment?
  5. Did you have an opportunity to interact with other patients?
  6. Would you stay have been better with access to the internet/email?
  7. Were you given adequate education about the equipment and supplies you were given?
  8. Did you receive all the equipment and supplies you needed for home?
  9. If you received outpatient services, were they started soon enough?
  10. Did anyone from the VA call to check on you after you were home?
  11. Would you recommend the rehab unit to others?
  12. Did you participate in at least one social activity per week?
  13. Were you involved with the decision-making regarding your rehab program?
  14. Did the rehab program prepare you for going home?
  15. Did the progress you made in rehab meet your expectations?
  16. Did the rehab program improve your health and quality of life?

The Rehab Team is committed to the principle that all patients, employees, volunteers and visitors deserve to be treated with dignity and courtesy and in a compassionate manner.

In all circumstances, we will treat patients in a manner that considers their background, culture, religion, heritage, and personal beliefs.

Every patient has the right to as much information as needed, or available, to make informed decisions regarding their care. Discharge planning will be coordinated with patients and families in order to meet the patient's continuing care needs.

Rehab staff will not have interests that conflict with their performance or result in private gain. All employees must abide by the federal rules for accepting or giving of gifts.

Patient information will not be shared in an unauthorized manner and patient privacy will be respected.

New services and techniques implemented will be based on such things as safety, effectiveness, efficiency, staff competency and research studies.

Patients may inquire about the business relationships of the medical center, affiliates and other healthcare providers that may influence the patient's care. In compliance with federal regulations, medical center staff will perform fair billing and conduct ethical business practices.
