The Day You Are Discharged Home

  1. Typically, patients are ready to leave the rehab unit BEFORE Noon on the day of discharge.
  2. Family training may be arranged depending on your discharge needs if not already completed.
  3. You will receive final medication teaching from a registered nurse or pharmacist.
  4. You must go to the pharmacy to pick-up your medications after your discharge.
  5. You will need to take the equipment home that has been given to you by the therapists.
  6. You will be provided with a list of rehab contacts for any concerns/problems after discharge.
  7. You will be informed of your future medical appointments at the VA. Appointments that are not scheduled by the time you leave will be mailed to you.
  8. You will receive any special discharge instructions that the rehab physician has written.
  9. You will need to sign discharge paperwork.
  10. You will be asked to complete a satisfaction survey.

After You Are Discharged

  1. Nursing will call you or your family/caregiver to ensure that:
    • you are adjusting well at home
    • your outpatient medical and therapy appointments are scheduled
    • you have received all equipment, medications and supplies that are needed
  2. You may be scheduled for an appointment to be seen by the rehab physician in approximately 4-6 weeks.
  3. Approximately 3 months after you have been discharged, an employee of Med-Tel Outcomes, Inc. will conduct a phone interview with you or your family/caregiver. The purpose of this phone call is to evaluate how well you are doing and provide the rehab team with information that will help us improve our services.
