Discharge

You will be discharged when there is a safe discharge plan in place (independent living, assisted living, long term care facility, etc.) and you have:
  • Achieved functional independence in all activities of daily living, or
  • Progressed to a state of functional independence with the help of trained caregivers, or
  • Reached a functional plateau in overall improvement, or
  • Shown significant slowing in rate of functional improvement.
If you become acutely ill during your stay and require medical or surgical services, you will be transferred to the appropriate service and discharged from the program.

If you choose to not participate in therapies or appointments or do not return from a therapeutic leave of absence on schedule, your rehab team will consider discharging you from the program. Your rehab doctor and social worker will discuss options with you to ensure a safe and appropriate discharge home or to another program. If you request to leave the hospital before completion of your rehab program and the team feels that you may be unsafe, your rehab doctor will talk to you about the risks and make arrangements for you to be discharged against medical advice.
