Personal Health Profile

It is important to keep track of your personal health information. You can do so in a folder, notebook, or even a piece of paper. Here are some topics you should consider including in your personal health profile:
  • Advance Directives (health care proxy, living will, etc.)
  • Allergies
  • Emergency contact information
  • Prosthetics devices (including eyeglasses, dentures, hearing aids, etc.)
  • Functional status (Are you able to move about on your own? What are your physical limitations?)
  • List of your health care providers
  • Hospital preferences
  • Immunizations
  • Insurance information
  • List of medical conditions
  • Medications and dosage amounts (prescription, over-the-counter, herbals, vitamins, etc.)
  • Vision and hearing status (How well can hear without your hearing aid? etc.)
  • Risk factors (Do you have a family history of diabetes? Heart disease? etc.)
  • Swallowing and/or dietary restrictions
  • Any other pertinent information that your health care providers should know about you

You can access trusted, secure, and current health and benefits information, as well as record and store important health and military history information. Veterans who receive health care at VA facilities are able to refill prescriptions and as additional features are released, you will be able to see key portions of your VA health record. There are even places for you to keep track of your personal health information.
