VISN 2: Veterans' Wellness, 2015 calendar
Personalized, Proactive, Patient-Centered Care and What It Means For You Through your service to our country, you have earned the right to high-quality health care. To deliver on that promise, VA Health Care Upstate New York has embarked on an ongoing mission to enhance the way it provides health care to Veterans.
Our quality initiatives are being carried out at a time when attitudes and expectations regarding health care are changing. In VA, it is now a team approach with Veterans as partners in their health care. Your Patient Aligned Care Team (PACT) is at the center of this effort and can help you plan your health goals for the coming year. I encourage you to further empower yourself by using My HealtheVet, which is an amazing online tool that offers a convenient way to learn more about your health care issues and concerns, including test results. It also provides you with 24/7 access to your PACT to address any health care issues you may have. To get started, register at www.myhealth.va.gov. Wishing you a happy and healthy holiday season and New Year, Darlene A. DeLancey, M.S. Interim Network Director |
