To improve the early identification and care of veterans in VISN 2, the Recognizing and Assessing the Progression of Cognitive Impairment and Dementia (RAPID) program promotes the early diagnosis of patients with dementia and other cognitive disorders, provides for the comprehensive assessment of the patients and caregivers, offers education and support to the caregiver over time, and provides for care management when appropriate.

Information and education is a critical component of the program. The patient and/or caregiver will receive contact information and Dementia prevention materials. It is believed that veterans and their families that have recently completed a dementia diagnostic work-up will be prepared and motivated to learn the importance of healthy behaviors in maintaining or improving cognitive function.
