Enrollment is an open process designed to provide access for veterans to a comprehensive package of VA health care services.
Any man or woman who was honorably discharged from the United States military prior to September 1980 is an eligible veteran. Individuals discharged after September 1980 must meet the following criteria:
Must have been honorably discharged
Must have completed at least two years of active duty (exceptions include Early Out, Hardship & Disability incurred or aggravated in the line of duty, completed term for which they were called).
How do I enroll?
You can enroll in person at any VA Medical Center or Clinic. Please refer to our Network 2 facility map for the location of the nearest facility.
You can also enroll by mailing or faxing the completed 10-10EZ form to the nearest Medical Center or Clinic. To speed the enrollment process please also mail or fax a copy of your DD-214 or Honorable Discharge Certificate.

What materials will I need for enrollment?
A copy of your DD-214 Report of Separation (or equivalent form). If you do not have access to this form, please see our Frequently Asked Questions for instructions on how to obtain a copy.
The enrollment system requires that most veterans provide the VA with a means test annually detailing their gross income and assets. This includes income for a veteran's spouse and dependent children.
Will there be any costs for health care?
Most veterans' care is free of charge, however some copayments do apply. See the Copayments and Charges page for additional information.
