Veterans Enrollment Guide

Who is eligible?
Enrollment is an open process designed to provide access for veterans to a comprehensive package of VA health care services.

Any man or woman who was honorably discharged from the United States military prior to September 1980 is an eligible veteran. Individuals discharged after September 1980 must meet the following criteria:

  • Must have been honorably discharged

  • Must have completed at least two years of active duty (exceptions include Early Out, Hardship & Disability incurred or aggravated in the line of duty, completed term for which they were called).

How do I enroll?
  1. You can enroll in person at any VA Medical Center or Clinic. Please refer to our Network 2 facility map for the location of the nearest facility.

  2. You can enroll electronically by completing the Online 10-10EZ form.

  3. You can also enroll by mailing or faxing the completed 10-10EZ form to the nearest Medical Center or Clinic. To speed the enrollment process please also mail or fax a copy of your DD-214 or Honorable Discharge Certificate.

  4. For assistance you may also contact your New York State or County Veteran Service Officer. For telephone listings please consult the blue pages of your local phone book under "New York State Offices," look for "Veterans Affairs".

What materials will I need for enrollment?
  • A copy of your DD-214 Report of Separation (or equivalent form). If you do not have access to this form, please see our Frequently Asked Questions for instructions on how to obtain a copy.

  • The enrollment system requires that most veterans provide the VA with a means test annually detailing their gross income and assets. This includes income for a veteran's spouse and dependent children.

Will there be any costs for health care?
Most veterans' care is free of charge, however some copayments do apply. See the Copayments and Charges page for additional information.

How can I obtain more information?

  1. Veterans with questions regarding benefits or ability to pay are encouraged to call our toll-free number Monday through Friday between the hours of 8:00 a.m. and 6:00 p.m.: 1-888-823-9656. We are able to answer all questions, often right over the phone.


  2. You can also visit your nearest local Veteran Service Center for assistance. Please refer to our Network 2 facility map for the location of the nearest facility.

About the Online 10-10EZ

For your convenience, Upstate new York Veterans can enroll electronically using the Online 10-10EZ Application for Medical Benefits.
  1. Submission of the online 10-10EZ application is fast and convenient and provides VA with the information to begin the enrollment process.

  2. Online submission of information takes place on a secure server for confidentiality and security.

  3. Each submission will be followed up with contact from a qualified veterans benefits counselor who will assist you in completing the enrollment process.
