Acute Rehabilitation Unit at Syracuse VA Medical Center

Nursing

The nurse is responsible for coordinating your nursing care and ensuring that the techniques learned in your therapy are carried over on the rehab unit.

The nursing staff will work with you in improving, restoring and challenging your independence and confidence. The nurses will focus on making you responsible for your own care including:
  • getting to your therapy appointments on time
  • requesting your medication as scheduled
  • managing your skin care
  • managing your bladder and bowel care
  • oral care, bathing, dressing and shaving
  • managing your prescribed diet
Social Work

The rehab unit social worker is responsible for coordinating a safe discharge. The social worker will meet with you and conduct an interview that will help identify your interests, needs and resources. This information will be very helpful to the rehab team in forming a discharge plan. The interview will include information about your living situation, financial and community resources, social supports, family, work, military history and your personal goals while on the rehab unit.

The social worker will arrange family meetings if needed and provide you and your family/caregiver with information about community resources and benefits that are available to you.

The social worker is also responsible for coordinating your transportation upon discharge and ensuring that follow-up appointments and services have been arranged.

Rehabilitation Coordinator

The rehabilitation coordinator assists the entire rehab team in providing a smooth and effective program. This person will:
  • assist in coordinating your admission, discharge and outpatient services
  • coordinate and document the rehab team meeting
  • review reports and monitor how well the rehab unit and staff perform
  • ensure compliance with various healthcare standards
Registered Dietitian

A registered dietitian is part of the rehab team and will be available as a consultant to:
  • recommend and provide special diets for patients with medical conditions requiring them such as diabetes, high blood pressure, heart disease, obesity, renal disease and malnutrition
  • provide nutrition education on special diets
  • evaluate menu plans and make adjustments per patient preferences and needs
